In preparation for your initial evaluation at DORSET PHYSIO or STRATTON PHYSIO, we use an online patient portal now for all patient registration forms found here: https://app.pteverywhere.com/s/dorsetphysiopt/register

If you prefer paper format, you may print and complete the following

Health History & Consent forms below for either the Dorset or Stratton location and bring them with you. 

PATIENT PAYMENT AGREEMENT FORM

Please arrive 15 minutes prior to your initial evaluation for registration.

You may be required to complete an outcome measure specific for your condition when you arrive.

Please bring a Photo ID, Insurance Card(s), your medication list, and a form of payment for any Payment Due and/or Supplies. Please wear loose comfortable clothing and supportive shoe wear that you would exercise in at each visit.

Visitor Policy:

You may bring 1 supportive family member or friend to your appointment if you need assistance in your care. A chaperone will be required for all minors for treatment provided to any sensitive areas for their safety.
